It is the responsibility of the North Mississippi Regional Center (NMRC) to inform the individuals we serve, as well as parents and/or legal representatives, of additional community-based services. The Mississippi Intellectual Disabilities/Developmental Disabilities (ID/DD) Waiver program provides additional service options for people who have intellectual/developmental disabilities. This program includes appropriate Medicaid funded services/supports to offer people with an ID/DD an opportunity to live at home with family or in a community setting as another option to care in an institutional setting. The Mississippi Department of Mental Health (DMH) is the operating agency for the ID/DD Home and Community Based Waiver Program often referred to as the ID/DD Waiver.
The NMRC Transitional Services Department employs a Transition Coordinator who serves as a guide to individuals and families to identify and secure community resources necessary to transition from an ICF/IID facility to a community setting of their and their family's choosing, utilizing the ID/DD Waiver. The Transition Coordinator assists the person and their family with the following: 1) Identifying requested supports and services, 2) Explaining the processes and procedures needed for movement to a more integrated setting, 3) Ensuring a successful transition to their least restrictive environment, and 4) Providing post-transition monitoring and reporting.
